HOW TO ORDER:

Mail or FAX the following order form to the address/ FAX number below.

Be sure to Include your FULL NAME, ADDRESS and TELEPHONE NUMBER (Including Area Code).  Be sure to include an ACTUAL STREET ADDRESS or ROUTE NUMBER as United Parcel Service will NOT deliver to Post Office Boxes. 

By including your telephone number, we will be able to contact you if there are any problems with your order. 

If your order is not received within a reasonable time, please feel free to contact us with the date you placed your order and the items ordered.

Send money order, cashier's check, certified check, personal check, or VISA / MASTERCARD / DISCOVER number.  Be sure to include all numbers including expiration date on credit cards.  There will be no additional charge for credit card orders.  *If using DISCOVER card, be sure to include three digit number from the back of the card.
